The problem

Exceptions surfaced in the ERP and then waited. A concentration breach or a cold chain excursion was visible the same morning it happened, but the decision about what to do sat in email until someone senior enough had time to read the thread. By the time it was answered the window had usually closed, and nobody could reconstruct afterwards who had decided what.

Deployment

Read-only connection in week one, no migration and no IT project. Policy thresholds configured against the operator's own margins rather than a benchmark. First governed decision routed in week four. Approvals stayed with the same named people who had always held them.
